On June 11, On Lok celebrated the grand opening of its new Program of All-Inclusive Care for the Elderly (PACE) Center in Union City, expanding access to comprehensive, community-based care for older adults across Southern Alameda County.
The new On Lok PACE center, which began serving participants earlier this year, is designed to help address a growing need for accessible senior healthcare services in one of the Bay Area’s fastest-growing and most diverse communities.
The 26,583-square-foot facility at 2600 Central Ave. can serve at least 400 older adults and their family caregivers through On Lok’s nationally recognized PACE model.
Elected officials, community leaders, healthcare partners and On Lok staff commemorated the opening.
“We’ve all seen the numbers: our country is aging and people are living longer. These PACE programs are going to make a huge difference in making sure that our loved ones are taken care of later in life,” said Union City Vice Mayor Jaime Patiño, who presented On Lok with a proclamation.
The need is significant. California’s older adult population is growing faster than any other age group, with nearly one in four Californians expected to be older than 60 by 2030. At the same time, many older adults face a fragmented healthcare system that can be difficult to navigate, particularly those with multiple chronic conditions and complex care needs.
The new On Lok Union City PACE Center reflects decades of lessons learned through On Lok’s experience caring for older adults and supporting family caregivers. Features include:
• An expansive adult day health center where participants can engage in wellness activities, receive rehabilitation therapies, enjoy nutritious meals and build meaningful social connections
• Dedicated memory care space to support participants living with cognitive challenges
• Nine spacious exam rooms designed to provide convenient access to coordinated vision, hearing, dental and podiatry services on site
• A computer and technology room that helps participants stay connected to family, friends and community resources
• A secured transportation area that helps ensure safe and reliable access to care
